Each year Progress Industries helps hundreds of persons who experience physical and mental disabilities to pursue their dreams and become more independent in the community. Admissions to Progress Industries are made through referral of the Department of Human Services, the Department of Vocational Rehabilitation, or through private application.
Eligibility
- Must have a documented mental or physical disability.
- Be sponsored by a funding source or be able to pay own fees.
- Be 16 years old for vocational services; 18 years old for residential services. (Children under 18 residing in parental home may be served in HCBS. )
- Be mobile (walk independently) or use wheelchair independently unless applying for ICF-MR.
- Be willing to participate in services.
- Not be unduly disturbing to others.
- Be no serious danger to self or others.
- Be free of communicable diseases.
- Must not be in active stages of alcohol or drug abuse (four months including treatment time).
- Must not be in need of greater services (medical/behavioral) than can be provided for by the facility.
- Must not have first or second degree criminal conviction within the past year.
- Will agree to be in evaluation for 30 to 90 days, depending on the needed time.
Admission Procedures
- Contact a funding/referral source or Progress Industries. A funding source could be the Department of Human Services, Vocational Rehabilitation, Veterans Administration, or private funding.
- Stop by one of our offices to fill out an application to provide us with the following information:
- Physical Examination (within the last 12 months)
- Social History
- Residential Intake Checklist (if applying for residential)
- Psychological or Psychiatric Reports (documentation of disability by M.D., Psychiatrist or Psychologist)
- Past hospitalizations per medical records.
- Educational Reports (IEP's, EBCE experience, etc.)
- Past Placement Information (vocational/residential)
- Guardianship, Custody or Court Order Documents (if applicable)
- A tour is welcomed and may be scheduled either before or after information is completed.
- After information is received, applicant will be interviewed, if considered appropriate.
- Accepted applicants are then placed on a waiting list until an appropriate opening occurs.
